Postendoscopic duodenal hematoma in children: Ultrasound diagnosis and follow‐up

Abstract: Intramural duodenal hematomas have most frequently been reported in children in a traumatic setting. We present two cases of duodenal hematoma that occurred after upper gastrointestinal tract endoscopy with biopsy in children without significant prior medical history. The diagnosis was made by ultrasound, in correlation with the clinical presentation. Because the patients were hemodynamically stable, they were treated conservatively and the regression of the hematoma was followed up with ultrasound until its c… Show more

“…However, due to the rarity of a post-endoscopic IDH and the missing prospective data, the incidence is of limited significance. With clinical suspicion of a duodenal hematoma, ultrasound or MRI should be performed to verify the diagnosis and to exclude differential diagnoses, such as a duodenal duplication, abscess, pancreatic pseudocyst or solid tumor [19]. A computed tomography (CT) is also described in several case reports, but MRI is superior due to the lack of ionizing radiation exposure [20] and superior soft-tissue resolution.…”

“…Наиболее информативными методами диагностики данной патологии являются УЗИ, рентгенография с барием и спиральная компьютерная томография с контрастированием, позволяющие определить локализацию, размеры забрюшинной гематомы, а также оценить состояние поджелудочной железы и соседних органов [14,15].…”

“…If there is no history of trauma, there is a high association with child abuse and additional imaging is warranted ( Figure 3). Hematomas may also result from endoscopic biopsy of the duodenum or in children with bleeding disorders [50][51][52] . Once identified, the hematoma can persist for at least two weeks, typically resolving by 6 wk.…”

“…Once identified, the hematoma can persist for at least two weeks, typically resolving by 6 wk. On ultrasound, duodenal intramural hematomas appear as a heterogeneous, nonvascularized mass along the course of the duodenum, which can obstruct the duodenal lumen and/or the common bile duct [50][51][52] . During resolution, the hematoma becomes cystic.…”
