Posterior pedicle screw fixation combined with transpedicular bone grafting for treatment of single-level thoracolumbar fractures with the aid of a vertebroplasty tool

Abstract: Objective This study was performed to assess the efficacy of a novel tool to assist transpedicular bone grafting in short-segment pedicle screw fixation combined with pedicle fixation at the level of the fractured vertebra (six-screw fixation). Methods We retrospectively analyzed 80 patients (40 in the control group and 40 in the tool-aided group) with single-level thoracolumbar fractures. Patients in the control group underwent traditional six-screw fixation combined with transpedicular bone grafting. In the … Show more

“…Additionally, the mechanical prying action of the "implantation hopper" can help reset the collapsed vertebral body, restore the height of the injured vertebrae, and achieve satisfactory correction of kyphosis, promoting bone healing. This can effectively reduce the neurological complications caused by bone defects and bone nonunion after surgery [30] . However, Kambin bone grafting has limitations such as puncturing into the injured vertebrae, redestruction of the vertebral bone cortex, cumbersome surgical steps, di culty in obtaining "bone grafting funnel" instruments, and prolonged surgical time.…”

“…Surgery can cause systemic stress response and muscle damage, which is a special stressor, and the CK levels of skeletal muscle, cardiac muscle, brain, and other tissue cells are significantly elevated, which can be used as an important indicator to determine muscle damage [ 22 ]. Patients experience significant muscle strain and fracture trauma during open arch root nail internal fixation surgery treatment, and plasma CK levels are significantly elevated [ 23 ]. Open arch root nail internal fixation surgical treatment requires paravertebral muscle distraction under the guidance of an automatic retractor, and paravertebral muscle ischemia is evident, which will further reduce the risk of lumbar back muscle ischemia and increase the pain level of patients [ 24 , 25 ].…”
