Posterolateral approach for humeral lateral condyle fractures in children

Abstract: We proposed a simple posterolateral approach to the articular surface of the trochlea for fresh and late fractures of the humeral lateral condyle. Twenty consecutive cases, 16 fresh and four old fractures, were approached between triceps and anconeus before the age of 15 years. All fractures achieved union in 3 months. Nineteen cases were classified as excellent or good by the Hardacre score after 31.3 months of follow-up. One late fracture with elbow subluxation showed radiographic fragmentation of the capite… Show more

“…Although open reduction and internal fixation through a posterolateral approach [ 14 , 34 ] or lateral approach (Kocher approach) [ 3 , 4 , 7 10 , 12 ] is the standard treatment for displaced LCFs, [ 13 ] these approaches are not appropriate for visualizing the fracture line on the capitellum (Milch type I LCFs). [ 30 , 31 ] Because the articular surface of the capitellum is present only on the anterior side, only an anterolateral approach permits restoration of joint congruity.…”

“…Because humeral lateral condylar fractures are intra-articular fractures, anatomical reduction of the cartilage of the distal humerus is essential. [ 2 , 3 ] Although the posterolateral approach, [ 5 , 6 ] Kocher approach, to humeral lateral condylar fractures is recommended, this approach does not permit visualizing fractures on the capitellum or the lateral edge of the trochlear groove, which have articular surfaces only on the anterior side (Fig. 4 A).…”

“…Lateral condylar fracture lines are classified according to Milch classification [ 4 ] : type I, fracture line lateral to the trochlear groove, and type II, fracture line into the trochlear groove. A posterolateral approach, [ 5 , 6 ] Kocher approach, is recommended for distal humeral lateral condylar fractures. This approach provides good access to the humeral trochlea from the posterior humerus and an optimal approach for open reduction and internal fixation (OR/IF) for lateral condylar fractures with trochlear fractures.…”
