Postoperative Endophthalmitis Caused by Cutibacterium (Formerly Propionibacterium) Acnes: Case Series and Review

Abstract: We report the clinical features, treatment strategies and outcomes in a series of patients with infectious endophthalmitis after cataract surgery caused by Cutibacterium acnes (C. acnes), formerly known as Propionibacterium acnes (P. acnes). This retrospective case series includes six eyes of six patients with chronic postoperative endophthalmitis caused by culture-proven C. acnesfrom December 2010 to July 2019 at a University referral center. “…More recently, the role of molecular testing by polymerase chain reaction or "PCR", is essential, and allows a microbiological diagnosis in 71% of cases of postoperative acute and delayed-onset endophthalmitis as demonstrated by Chiquet (37)(38)(39). However, diagnosis based on clinical ndings should be performed, without waiting for the microbiological results the to treat (22,29). E.faecalis was the only identi ed microorganism in our study.…”

“…Cutibacterium acnes (previously known as Propionibacterium acnes ) is known to cause an indolent late-onset postoperative endophthalmitis[ 21 ] and has been cultured from CBS fluid in eyes without other evidence of inflammation. [ 22 , 23 ] While C. acnes endophthalmitis has been reported following Nd: YAG laser posterior capsulotomy, these studies included eyes with PCO and without CBS.…”

“…Stenotrophomonas maltophilia and Corynebacterium macginleyi were detected in multiple cases of bacterial keratitis ( Cho and Lee, 2021 ; Sagerfors et al., 2021 ). Bacillus cereus , Enterococcus faecalis , Staphylococcus haemolyticus and cutibacterium acnes are considered as opportunistic pathogens of endophthalmitis ( Panda and Singh, 2018 ; Ehling-Schulz et al., 2019 ; Chen et al., 2021 ; Fowler et al., 2021 ). In the culture results of the control group, only two opportunistic pathogens were found, among which Acinetobacter lwoffii could induce endophthalmitis after cataract surgery ( Roy et al., 2015 ).…”

“…In the culture results of the control group, only two opportunistic pathogens were found, among which Acinetobacter lwoffii could induce endophthalmitis after cataract surgery ( Roy et al., 2015 ). The other, cutibacterium acnes , can cause postoperative endophthalmitis ( Fowler et al., 2021 ) and keratitis ( Ovodenko et al., 2009 ), but its detection rate on the ocular surface of TAO patients is significantly higher. In previous studies of TAO-exposed keratitis, successful bacterial culture include Pseudomonas sp., Acinetobacter sp., Corynebacterium sp., Escherichia coli , Streptoccocus pneumoniae , Staphylococcus aureus and Staphylococcus epidermidis , some of which were consistent with the results of sequencing and culture in this study ( Naik et al., 2019 ).…”
