Postoperative incidence of incarcerated hiatal hernia and its prevention after robotic transhiatal esophagectomy

Abstract: The results indicate that postoperative incarcerated hiatal hernia after RE is an infrequently reported, albeit serious, complication. A preexisting hiatal hernia may put patients at a higher risk of incarceration. According to the authors' experience, a primary closure and reinforcement with mesh sutured to the gastric wall is recommended as a preventive measure. Diligent follow-up evaluation with regular computed tomography (CT) scans investigating likelihoods for incarceration is advisable, especially for p… Show more

“…Following a trans-hiatal esophagectomy, a large diaphragmatic defect may be closed, while the abdomen is still open; either by suturing the crural defect or suturing the gastric conduit to the diaphragmatic edge [14]. Other surgeons advocate the use of a biological mesh to close the hiatus to prevent this problem [16]. If these techniques are employed, it is important not to compromise the blood supply to the gastric conduit.…”

“…The long-term results of MIE are just beginning to be documented, including potential complications of DH [2][3][4][5][6][7]. The overall incidence of DH may be underestimated given the limited follow-up and high disease recurrence of the post-esophagectomy patient.…”

“…The overall incidence of DH may be underestimated given the limited follow-up and high disease recurrence of the post-esophagectomy patient. DH may occur at a higher incidence after MIE when compared to open procedures [2][3][4][5][6]. An evaluation of our institution's incidence of post-MIE DH was performed in addition to a review of the published literature regarding DH after both open and minimally invasive esophagectomy.…”

“…To the Editor, We are grateful to Dr. Boone and colleagues for their insightful comments regarding our report on the incidence of incarcerated hiatus hernia after robotic-assisted transhiatal esophagectomy [1]. We agree with Dr. Boone and colleagues that a complication of an incarcerated hiatal hernia should not be induced by a transthoracic approach, as the integrity of the hiatus is maintained by this surgical technique.…”
