Posttraumatische Oberlidretraktion

Abstract: Three unusual cases of posttraumatic upper eyelid retraction are described. They were observed following orbital trauma without apparent involvement of the superior rectus muscle after latency periods ranging from several months to years. As other reasons for the lid retraction were ruled out the question arises as to whether peripheral neurogenic or muscular upper eyelid retraction must be assumed to be a consequence of trauma, although this has never been described before.

“…Upper eyelid retraction is an ophthalmological condition most frequently associated with Graves ophthalmopathy 14. Other published reports describe upper eyelid retraction associated with blow-out fracture of the orbit,1517 following periorbital contusion without fracture,18 after ocular surgery,19 in patients with neurogenic disease20 and associated with contralateral blepharoptosis 21. Cruz et al reported two cases of unilateral upper eyelid cicatricial retraction in patients with encephalocraniocutaneous lipomatosis.…”

“…For that reason, upper eyelid retraction is a paradoxical response. There are few reports on posttraumatic upper eyelid retraction with latency periods ranging from several months to years 4-6. Putterman et al4 reported upper eyelid retraction after a blow-out fracture, and hypothesized that lid retraction occurs secondary to overaction of Müller's muscle.…”

“…In contrast, Conway5 reported upper eyelid retraction following a blow-out fracture of the orbit with enophthalmos and postulated that this was due to traction on the connective tissue sheath of the levator palpebrae muscle through its connective tissue interconnections with other extraocular muscles. Hatt6 also described three cases of posttraumatic upper eyelid retraction without apparent involvement of the superior rectus muscle after latency periods ranging from several months to years; however, he was uncommitted regarding the mechanism of eyelid retraction.…”
