Potential salvage therapy for inadvertent intrathecal administration of vincristine

Abstract: Summary.Vincristine is an anti-neoplastic agent for intravenous use. Inadvertent intrathecal administration of vincristine tends to follow a predictable clinical sequence of a progressive ascending radiculomyeloencephalopathy, usually fatal in outcome. We report a case of a 10-year-old girl who suffered an inadvertent intrathecal vincristine administration. We have outlined the management strategy used and her consequent neurological progress. She has survived but with a significant neurological deficit. Her o… Show more

“…There are 7 reported cases in which an intrathecal vincristine injection was mistakenly given, which then received CSF irrigation fluid plus fresh-frozen plasma following CSF aspiration to try to counter the effects of vincristine [2,6,10,11,12,13,14]. Six of the 7 survived, although all had paraparesis or tetraparesis [6,10,11,12,13,14]. There are no survival reports of patients mistakenly receiving an intrathecal vincristine injection who did not have CFS irrigation with fresh-frozen plasma.…”

“…There are no survival reports of patients mistakenly receiving an intrathecal vincristine injection who did not have CFS irrigation with fresh-frozen plasma. It is believed that adding fresh frozen plasma to the CSF causes rapid binding of the vincristine to blood components and thus minimizes its neurotoxicity [6,12]. In addition to aggressive CSF dilution, administration of neuroprotective agents such as pyridoxine, folic acid, glutamic acid, and corticosteroids has been widely used to prevent further neuronal damage from vincristine [15].…”
