Potentially Inappropriate Medications Are Associated with Increased Healthcare Utilization and Costs

Abstract: BACKGROUND/OBJECTIVES: To examine the prevalence of potentially inappropriate medication (PIM) prescribing and its association with healthcare utilization and related expenditures utilizing nationally representative data from the United States. DESIGN: Retrospective cohort study. SETTING: The 2011-2015 Medical Expenditure Panel Survey (MEPS). PARTICIPANTS: Community-dwelling sample of U.S. adults aged 65 and older during the first round of each MEPS cycle. MEASUREMENTS: A qualified definition operationalized f… Show more

“…In other studies conducted worldwide for the use of PIMs according to 2019 AGS Beers Criteria, the prevalence ranged from 34.1 to 68.8%. These studies were conducted in Lebanon (34.1%) (Chahine 2020), in USA (34.4%) (Clark et al 2020), in China (35.0%) (Huang et al 2020), in Switzerland (53%) (Achterhof et al 2020), and in Spain (68.8%) (Lopez-Rodriguez et al 2020). The difference in prevalence is predicted and comparing other studies should be made with caution due to differences in population studied, time of the study, source of patient information, healthcare setting, and medication availability and accessibility in different countries.…”

“…Recent studies highlighted that over a third of seniors aged 65 years and older are being prescribed potentially inappropriate medications (PIM) that are associated with adverse events that lead to increased healthcare utilization and treatment costs. 38,39 These included both acute and chronic medications, and were associated with a 21% increased risk of experiencing an adverse drug event, having to visit the emergency department, or being rehospitalized. 38,39 Therefore an additional group of medications were added to the HR medication list for those patients over the age of 65, largely incorporated from the 2019 BEERS Criteria, STOPP criteria, Choosing Wisely statements, and Kim et al [40][41][42][43] These potentially inappropriate medications in the elderly included endocrine, anticholinergic, proton-pump inhibitors (PPIs), electrolyte disturbing, and increased fall/fracture risk medications.…”

“…PIMs have been associated with falls, confusion, death, and hospitalizations in older community dwelling populations 4,5 . PIMs use in older adults is also associated wither higher healthcare utilization, including outpatient visits and emergency room visits 42 . Notably, more than half of older adults with CKD have at least one PIM 43 .…”
