Povidone-Iodine sclerosis of pelvic lymphoceles: A prospective study

Abstract: Twelve patients presented for percutaneous catheter drainage of 13 postoperative pelvic lymphoceles. Six patients with 7 lymphoceles were treated with povidone-iodine sclerosis prior to catheter removal. Only 1 lymphocele (which continued to drain large amounts of fluid during and after the sclerosis procedure) recurred. Six patients with 6 lymphoceles had their percutaneous catheters removed without sclerosis. Fluid collections recurred in 3 instances, necessitating repeat percutaneous drainage (2 patients) o… Show more

“…The mechanism of action of povidone-iodine appears to be related to enhanced sclerosis, 73 although the precise mode of action remains unclear. Iodine has strong oxidative and cytotoxic properties, which induces a potent inflammatory response in the wall of any fluid-containing structure.…”

“…Thus, instillation volume, frequency of passes and duration of treatment vary ac cording to the author [6,[8][9][10][11], We use a standard instilla tion volume (50 ml) without taking into account the drained volume. Standardization of the method needs more series than those reported until now.…”

“…Gilliard et al [52] report that 8 of 9 pre viously untreated lymphoceles were successfully treated with installation of 10% povidone iodine. Many other agents are reported in the literature: tetracycline; doxycycline; sodium tetradecylsulfate; sodium morrhuate; ampicillin, and fibrin [16,44,[53][54][55][56], The success of sclerosis in refractory lymphoceles does not appear to be as effec tive as in virgin lymphoceles. Pollack et al [46] treated six lymphoceles refractory to percutaneous drainage with tetracycline.…”
