Practical Means of Preoperative Diagnostics of Primary Fallopian Tube Cancer

Abstract: The aim: To analyze contemporary practical means to improve diagnostics of primary fallopian tube cancer. Materials and methods: Authors analyzed specifics of clinical signs and anamnesis in 152 PFTC patients. Diagnostic capacity of cytological analysis of pathologic vaginal discharge, X-ray contrast methods of examination, sonography, tumor markers, and computed tomography was studied. Own results of PFTC diagnostics using different methods and world practice using MRI, PET-CT and laparoscopy were discussed. … Show more

“…The use of FDG-PET/CT (fluorodeoxyglucose-positron emission tomography/computed tomography) has been tested in the search and evaluation of incomplete tumor removal; however, the data to date cannot support the examination as a routine diagnostic examination in daily clinical practice [ 25 ]. A recent study suggests that complex examination, including cytological examination of abnormal vaginal discharge, tumor markers, computed tomography, magnetic resonance imaging, PET-CT, and laparoscopy, may significantly help in the diagnosis and to avoid inadequate surgery [ 26 ].…”

“…Approximately 70% of patients with PFTC have pelvic inflammatory disease, and 49% complain of infertility according to the patients’ history and intraoperation revision data. 7 The incidence rate of PFTC is 0.36 to 0.41/100,000 women each year. However, from 2001 to 2014, the incidence of PFTC increased 4.19 fold.…”

“…A typical feature of FTC is the correlation between pain and discharge: increased pathological discharge after an episode of pain. Bimanually, at the time of consulting a gynecologist, 70 to 76% of patients already present with anatomical changes in the internal genital organs, mainly the uterine adnexa, which is often a sign of an old disease [1,20].…”

“…The errors were probably caused by the lack of the following: developed laparoscopic semiotics of FTC, proper examination of the longitudinal section of the removed fallopian tube in the operating room, and suboperative morphological diagnosis. Despite the complexity of preoperative diagnosis of FTC, clinical manifestations and comprehensive use of additional modern examination methods make it possible to avoid mistakes in most cases [20,21,32,37].…”
