Practice Preferences for Glaucoma Drainage Device Implantation and Cyclodestruction in Australia and New Zealand

Abstract: A wide range of practice patterns for GDD implantation and cyclodestruction exist among Australian and New Zealand ophthalmologists. This likely reflects a paucity of good evidence to guide practice. The Australasian indications for GDD implantation were moderately different from the reported practice in America. Well-designed clinical trials are needed to better define the indications and best practice for these 2 important glaucoma treatment modalities.

“…GDIs have been demonstrated to be effective in reducing intraocular pressure (IOP) and are especially indicated in cases with high risk of failure after filtering procedures [3,4]. Typically, GDIs are indicated for eyes that have already undergone one or more glaucoma surgeries, and for particular types of glaucoma in which filtering procedures are likely to fail (e.g., secondary glaucomas) [5][6][7][8]. The use of GDIs as a primary surgical procedure for glaucoma has also been investigated [9,10].…”

“…[23] However, in our poll, only 28% glaucoma specialists reported doing GDD implantation. On similar lines, nearly half (49%) glaucoma specialists in Australia and New Zealand[24] and 40% glaucoma specialists in Canada[11] did not perform GDD implantation surgery. Moreover, 76% of GDD surgeons in Australia and New Zealand did not perceive an increase in their volume of GDD implantation over 5 years preceding their survey.…”

“…They are considered if one or two trabeculectomies with MMC fail or if extensive conjunctival scarring exists. [51] Some authors who have favorable outcomes with glaucoma drainage implant select it as the procedure of choice in uveitic glaucoma. [52], [53] Two types of drainage implants exist.…”
