Practices and Attitudes of Michigan-Based Occupational Physicians Regarding Adult Immunization

Abstract: Occupational physicians frequently recommend influenza, tetanus, and hepatitis B vaccines when indicated, but are less likely to order other vaccines for patients. Promotion of a more comprehensive assessment of immunity needs in the workplace may improve national vaccine coverage.

“…Until recently, despite the underlying legal framework, and the substantial burden represented by pathogens such as HBV, HCV, and HIV, the formal education and the medical training of Italian OPs have often prioritized other topics (e.g., work-related musculoskeletal diseases, occupational pulmonary diseases, occupational neoplasia) over biological risk [15,37,39,68]. In other words, despite their professional role, scientific background, and medical training, not only may OPs fail to overcome the gaps between official recommendations and vaccine objectors [1,2,7,67,69], but their knowledge gaps could even lead to a certain degree of vaccine hesitancy [2,6,7,53,59].…”

“…Another explanation of the participation of OPs in the preventive measures for LD may be found in the relatively low estimates for risk perception. The very low share of participants acknowledging the high frequency of LD in occupational settings not only reasonably reflects the common knowledge framework on LD [ 20 ], but could represent a potential consequence of the information otherwise conveyed by new and conventional media, a particularly critical issue when dealing with the knowledge status and risk perception of OPs [ 58 , 64 , 66 , 67 , 68 , 69 , 70 ]. Legionella infections have repetitively evoked both media and public interest, which in turn has resulted in high pressure on medical professionals.…”

“…The first has been defined as a self-limiting febrile flu-like syndrome, while LD is a severe pneumonia that can be complicated by significant extra-pulmonary manifestations, such as renal failure or pericarditis [2,3]. To date, genus Legionella encompasses a total of 60 species and almost 2 of 27 70 distinct serogroups. Even though most human cases in western countries have been associated with the L. Pneumophila serogroup 1 (Lp1) [4], the share of cases associated with the non-Lp1 serogroup has recently increased [1][2][3]5,6], especially in Australia and New Zealand [7].…”
