Praxis der Tumorendoskopie an deutschen HNO-Kliniken

Abstract: Practice of tumour endoscopy in German ENT-clinics is widespread but does not follow standardised mechanisms. Current international literature shows that there is no common consensus on value and techniques of tumour endoscopy, however, due to highly developed radiological diagnostics, risks of rigid endoscopies and unknown incidence of second primary tumours it is discussed more and more negative. To establish future guidelines, controlled studies or analysis of large populations seem to be necessary.

“…So the value of endoscopy is rather low regarding the question of secondary carcinoma in those cases. In Germany, the performance of tumor endoscopy with rigid instruments is still widespread 154 . As long as there is no reliable evidence, endoscopy may be performed in the current standardized way, however, for HPV OSCC also system oriented biopsy under general or local anesthesia can be performed without any concern.…”

“…Additionally, as radiological diagnostic means are improving, the significance of triple endoscopy is often questioned. However, to date, pre-therapeutic triple endoscopy can be regarded as the standard-of-care in German tertiary care ENT centres [2]. Alternatively, symptom-directed triple endoscopy is increasingly discussed as a valid alternative, particularly for SSPC detection [10].…”

“…Head and Neck squamous cell carcinoma (HNSCC) patients present with simultaneous second primary carcinoma (SSPC) in up to 4% of the cases at the time of diagnosis [1,2]. Direct endoscopy of the upper aerodigestive tract is frequently conducted to detect HNSCC because clinical examination alone can be insufficient [3].…”

“…Overall 75% of these suffer a relapse within 2 years. While the complete resection of the tumor is still investigated by histopathological evaluation, precancerous lesions are often overlooked or tumor borders are delimited inaccurately . The spatial characterization of the different tumor parts is mainly investigated on the genetic level .…”
