Pre‐emptive renal transplantation in children

Abstract: Background: Renal transplantation is a definitive therapeutic modality in end-stage renal disease (ESRD). Most ESRD patients in Japan experience dialysis prior to renal transplantation. The present study was undertaken to examine the usefulness of pre-emptive renal transplantation (PET). Methods:Between 1987 and 1998, 255 renal transplantations were carried out by the authors. Among those consecutive cases, 10 were cases of PET. In nine pediatric cases, demographics, graft and patient survival, height growth a… Show more

“…In terms of preemptive RTx, 25% of patients in G1 and G2 and 17% in G3 received a preemptive graft; these percentages are higher than those reported by Cransberg et al in Europe (ie, 14%, with 20% of DD) . Even though the effect of preemptive RTx on graft survival differs between studies, we were unable to demonstrate any benefit of preemptive RTx in the present series.…”

“…More than half of the participating patients, who were at that time on dialysis, had previously undergone transplantation and had experienced graft rejection. Pre-emptive transplantation results in improved growth and better psychosocial development [32,33], although we did not find better scores of HRQOL in this group of transplant patients. The use of a specific HRQOL instrument would be better suited to detect subtle changes and specific symptoms that modify HRQOL.…”

“…9–14 However, the benefits of completely circumventing dialysis for children with ESRD have not been clearly demonstrated. Several studies have demonstrated no differences in patient or graft survival between children receiving preemptive vs. non-preemptive transplant 1521 and there is some evidence to suggest discrepancies in the graft survival benefits of preemptive transplants by donor source. 2224 Importantly, the majority of these studies have been limited by small sample size and few events.…”

“…2224 Importantly, the majority of these studies have been limited by small sample size and few events. 1524 We hypothesized that given a large, national sample of children with ESRD followed over a decade, we would observe significant patient and graft survival benefits of preemptive vs. non-preemptive kidney transplantation and would be able to discern whether any differences exist across donor source.…”
