‘Pre-endoscopy point of care test (Simtomax- IgA/IgG-Deamidated Gliadin Peptide) for coeliac disease in iron deficiency anaemia: diagnostic accuracy and a cost saving economic model’

Abstract: BackgroundInternational guidelines recommend coeliac serology in iron deficiency anaemia, and duodenal biopsy for those tested positive to detect coeliac disease. However, pre-endoscopy serology is often unavailable, thus committing endoscopists to take routine duodenal biopsies. Some endoscopists consider duodenal biopsy mandatory in anaemia to exclude other pathologies. We hypothesise that using a point of care test at endoscopy could fill this gap, by providing rapid results to target anaemic patients who r… Show more

“…In our study, this practice only increased diagnostic yield by 2.4% and two-thirds of these patients were referred for OGD specifically for biopsy (five with positive endomysial antibodies and one patient with dysphagia for oesophageal biopsies to diagnose eosinophilic oesophagitis). Based on previous local costings at our institute,21 this approximates to a total cost of over £21 000 (£10 100 for biopsy forceps, £10 600 for histology and £1100 for rapid urease testing). These data would support the findings of Nelsen et al, who found that a practice of having a low threshold to take biopsies for histology had a low yield at high cost 22…”

“…A biopsy may be indicated anyway, even with a negative TTG. 31 IgA deficiency is relatively common in the population and, if present, TTG measurement may give a falsely negative reading.…”
