AM local time, an explosion occurred in the Zemestan-Yurt coal mine in Azadshahr City of Golestan Province, northern Iran, which resulted in mass casualty incident. Materials and Methods: In total, more than 126 miners were affected. Of them, 43 miners were killed, 73 other workers who helped their coworkers were also injured and taken to the hospital, and 10 injured people received on-site prehospital medical care. Many defects in the response phase were identified in the scene management and the triage. Results: It seems necessary to establish a response plan for mass casualty incident, hold regular education, and plan to prepare involved units for appropriate responses. Conclusion: These measures are the first steps needed to improve preparedness and response to a mass casualty incident.

RESUMO Objetivo: descrever estratégia de ensino a partir da simulação de Incidente de Múltiplas Vítimas (IMV), discutindo e avaliando a atuação dos discentes envolvidos no atendimento inicial às vítimas de trauma. Métodos: estudo transversal com abordagem quantitativa que contemplou a execução de uma simulação realística de IMV, envolvendo discentes, docentes dos Cursos de Medicina e de Enfermagem, além de profissionais do atendimento pré-hospitalar. Resultados: a partir da análise de 17 checklists, foi possível perceber que a classificação segundo o método START (Simple Triage And Rapid Treatment) aconteceu de forma correta em 94,1% dos atendimentos. Seguindo a avaliação primária com o mnemônico ABCDE, todas as etapas foram realizadas de forma correta em 70%. Contudo, só houve oferta de oxigênio em alto fluxo em 64,7% dos atendimentos. A pesquisa por fontes de sangramento visíveis e ocultas foi realizada em 70,6% dos atendimentos. A avaliação neurológica com a escala de coma de Glasgow e avaliação pupilar ocorreu em 70,6% das vítimas. A exposição da vítima foi realizada em 70,6% dos atendimentos. Conclusão: ambientes simulados permitem a consolidação e o aperfeiçoamento de competências e habilidades profissionais, principalmente quando se trata de uma área pouco treinada na graduação, como o IMV. O treinamento precoce e o atendimento em equipe estimulam o raciocínio clínico, a integração e a comunicação, aspectos essenciais diante de situações caóticas.

Integration in operations between 1990 Suwa Seriya ambulance service and A&E services of hospitals in Sri Lanka was assessed qualitatively using the Mini Focus Group Discussion (Mini-FGD) technique and applying a Likert scale survey. Agreed group-perceived satisfaction of the A&E staff was recorded based on identified thematic areas. The technique of patient handling, reliability of vital sign recording, providing random blood sugar reading, and professional relationship with hospital staff of Suwa Seriya emergency ambulance service were the thematic areas that recorded positive agreed group-perceived satisfaction indicating a potential to broaden the integration process with the A&E operations of the country. Quality of providing clinical history, administration of medication, and awareness of A&E operations of emergency ambulance service indicated least agreed group-perceived satisfaction. Moreover, providing clinical notes and investigation records such as electrocardiogram, sharing feedback about the patient, prehospital triaging, and pre-alert system were found to be non-operational. The health-care policymakers of Sri Lanka should exploit the positives and explore the negatives of the A&E staff perception on 1990 Suwa Seriya ambulance service, focusing on widening the integration process.
