Predicting Risk of Urinary Incontinence and Adverse Events After Midurethral Sling Surgery in Women

Abstract: OBJECTIVE: To construct and validate models that predict a patient's risk of developing stress and urgency urinary incontinence and adverse events 12 months after sling surgery. METHODS:This was a secondary analysis of four randomized trials. Twenty-five candidate predictors (patient characteristics and urodynamic variables) were identified from the National Institute of Diabetes and Digestive and Kidney Diseases Trial of Mid-Urethral Slings (N5597). Multiple logistic models were fit to predict four different … Show more

“…age, body mass index, performing anterior colporrhaphy) and developed an excellent online calculator to predict a woman's individual risk to have postoperative SUI. This study group developed another appealing online calculator for balancing risks and benefits for women considering MUS in SUI without prolapse . A similar tool would be helpful in considering combination surgery in women with pelvic organ prolapse.…”

“…26 The model had a high concordance index and outperformed expert opinion and cough stress test results alone at predicting latent SUI. 26 The calculator is available online (riskcalc.org/FemalePelvicMedicineandReconstructiveSurgery/). Sharing a patient's individualized risk of latent SUI following prolapse repair can help tailor the decision of whether or not to perform a prophylactic anti-incontinence procedure, when balanced with a discussion of the surgical risks and the patients' preferences.…”

“…An additional important contribution of these trials was the introduction of a risk calculator for determining the rate of subjects who will report SUI following a prolapse corrective procedure. The authors used the OPUS trial data to construct a model for predicting the risk of latent SUI following prolapse repair, which they validated using the CARE trial data . The risk calculator includes seven variables: age, vaginal parity, body mass index, preoperative reduction stress test, performance of concurrent anti‐incontinence procedure, urgency urinary incontinence symptoms, and diabetes .…”
