Background: Despite excellent results from free flap transfers, postoperative thrombosis does occur and, although infrequent, causes total loss of the flap. We investigate the risk factors of anastomotic thrombosis in head and neck free flaps. Study Design, Subjects and Methods: A retrospective analysis was carried out on 200 consecutive free flaps applied to patients who underwent head and neck reconstruction. Multiple logistic regression analysis was conducted to identify risk factors of anastomotic thrombosis, taking into account twelve factors: diabetes, obesity, hypertension, hypertriglyceridemia, proteinuria, age, gender, smoking habit, habitual alcohol intake, previous chemotherapy and radiation, and hypercholesterolemia. Results: Hypertension, female gender, aging, and drinking habit were statistically significant risk factors of postoperative thrombosis. Conclusions: Our data suggest that four factors mentioned above are possible predictors of anastomotic thrombosis in free flaps used for head and neck.

В отделении онкологии и детской хирургии ФГБУ «НМИЦ ДГОИ им. Дмитрия Рогачева» Минздрава России за 2015-2017 годы проведе-ны 14 операций по поводу доброкачественных и злокачественных новообразований нижней и верхней челюстей, мягких тканей височной области, внеорганных опухолей мягких тканей головы. При этом одномоментная или отсро-ченная реконструкция выполнялась костно-мы-шечным лоскутом гребня подвздошной кости, малоберцовым лоскутом или мягкотканным лучевым лоскутом. Во всех случаях анастомозы оказались состоятельны, лоскуты жизнеспо-собны. В статье приведен клинический пример пациентки Т. 6 лет с диагнозом центральной ги-гантоклеточной гранулемы тела нижней челю-сти. Пациентке выполнено удаление опухоли с симультанной реконструкцией свободным малоберцовым лоскутом с использованием стереолитографических резекционных шабло-нов. Длительность наблюдения пациентки на момент написания статьи составляет более 1,5 года. Рецидива заболевания не выявлено, свободный лоскут состоятелен и функциона-лен, ребенок социально адаптирован. Учитывая радикальность удаления первичной опухоли, сохранение иннервации мягких тканей подбо-родочной области, полную подвижность мими-ческой мускулатуры лица, быструю социальную адаптацию ребенка, функциональную сохран-ность нижней челюсти и способность питания через рот, а также возможность зубного про-тезирования в послеоперационном периоде, метод лечения, выбранный в данном клини-ческом случае, можно считать оптимальным. Применение нами свободных лоскутов было оптимальным и во всех остальных случаях бла-годаря таким преимуществам, как выполнение высокорадикального лечения на резекцион-ном этапе операции, хорошие функциональ-но-эстетические результаты, быстрая социаль-ная адаптация детей. Расширение показаний к проведению реконструкции свободными ло-скутами представляется перспективным в пе-диатрической практике. У взрослых пациентов свободные лоску-ты для замещения значительных дефек-тов головы и шеи используются с конца XX века. Эта техника подтвердила свою эффективность в реконструктивной хирургии и детского возраста [1,2]. Ее применение у детей и подростков оправдано благодаря наличию соб-ственного кровообращения и возможности ис-пользования единого реконструктивного мате-риала в течение всей жизни пациента по причине роста и адаптации лоскута в реципиентной об-ласти в процессе взросления [3]. Широкое разно-образие свободных лоскутов, различающихся по тканевому составу, функциональной нагрузке и длине сосудистой ножки, обеспечивает боль-шой выбор оптимального материала для рекон-струкции смешанных дефектов значительных размеров. В онкологической практике к преиму-ществам метода следует отнести возможность дальнейшего проведения химио-или лучевой Альманах клинической медицины. 2017 Сентябрь-октябрь; 45 (6): 518-524.

Despite their reliance on small-vessel anastomoses for survival, free flaps are extremely reliable with regard to the incidence of flap necrosis, which contributes to a low incidence of perioperative complications. Selection of flaps that have proven dependability contributes to a successful outcome. While this technique frequently requires lengthy surgery in an elderly patient population, the perioperative mortality) and morbidity are acceptable. Because of their unsurpassed reliability, free flaps have become the preferred method of reconstruction for most patients with major defects in the head and neck region.
