Goldberg et al (1977) have similarly reported that major role therapy, combining social casework and vocational rehabilitation, can have antitherapeutic effects in some pa tients; their conclusion, that patients can be provoked into a mental state they can no longer manage, resulting in panic and exacerbation of symptoms, is clearly consistent with our model.
