Predictive factors by multivariate analysis for colorectal cancer in Malaysian patients undergoing colonoscopy

Abstract: With the ever-increasing demand for gastrointestinal endoscopy, the appropriate utilization of colonoscopy is essential to afford prompt patient evaluation. Our study supports the need to prioritize the use of colonoscopy in patients with rectal bleeding and iron deficiency anemia. In the older patient where the background prevalence of colorectal cancer is higher, referral for colonoscopy is also justified.

“…About 93% were done for investigation of symptoms and surveillance of previous cancer. This shows that the use of colonoscopy as a screening tool in asymptomatic patients is underutilised (Tan et al, 2002). Among Malaysian patients, local study shows that anaemia is the most common presentation (Rashid et al, 2009).…”

“…Based on the above results, the authors suggest that colonoscopy should be prioritized to the high risk group population, especially those with positive family history and older age group. Tan et al also suggested that priority given to patient presented with rectal bleeding and iron deficiency anemia (Tan et al, 2002). Simultaneously, concerted effort is needed to increase numbers of skill operators and availability of the procedure throughout the country.…”

“…Rectal bleeding is the most common and important symptom of CRC (Majumdar et al, 1999;Tan et al, 2002) with the highest positive predictive value for CRC than other symptoms (Ferraris et al, 2004). Hence, awareness on this symptom will be crucial to promote early identification of the disease.…”
