Predictive factors of clinical success after adrenalectomy in primary aldosteronism: A systematic review and meta-analysis

Abstract: BackgroundUnilateral adrenalectomy is the mainstay treatment for unilateral primary aldosteronism (PA). This meta-analysis aimed to systematically analyse predictors of clinical success after unilateral adrenalectomy in PA.MethodsA search was performed using PubMed/Medline, Scopus, Embase and Web of Science from their inception to February 2022. Observational studies in adult PA patients which reported predictors of clinical success after unilateral adrenalectomy were included. A random-effects model was emplo… Show more

“…Patients with adrenal adenomas were the likeliest to be cured in our study. Wang et al (18) found the same result as we did, Celen et al (16) found that persistent hypertension after adrenalectomy was only favored by micronodular hyperplasia, while Proye et al (26) did not found a statistically significant association between pathologic findings and hypertension cure. These results should prompt us to warn patients, in whom the diagnoses of hyperplasia has been confirmed by examination of the operative specimen, of the possibility of persistent hypertension and that complete resolution of hypertension is more unlikely.…”

“…Indeed, many studies have suggested that patients are more likely to become normotensive after adrenalectomy if they are aged 50 years or younger and have had hypertension for less than 5 years at the time of surgery ( 12 , 14 , 17 , 18 ). It has been suggested that long-lasting hypertension (despite surgical correction of hormonal abnormalities) causes irreversible vascular damages, which is the main reason for explaining why prolonged hypertension is a risk factor for surgical outcome ( 19 ).…”

“…23 Furthermore, a multicenter study showed that young patients were more likely to benefit from surgical treatment. 24,25 This favorable overall outcome in young patients with PA probably reflects the beneficial effect of early detection of PA and early administration of disease-specific treatment for the disease. The strong association between cardiovascular comorbidities at baseline and mortality also supports the importance of early detection and treatment of PA.…”
