Predictive utility of cardiac ultrasound in traumatic cardiac arrest in a combat casualty

Abstract: We report a case of traumatic cardiac arrest in a combat casualty who was resuscitated to return of spontaneous circulation despite asystole and no visible cardiac activity on initial ultrasound examination. This return of spontaneous circulation suggests that survival may be possible in traumatic cardiac arrest due to exsanguination, even when there is no demonstrable cardiac activity on ultrasound. Cardiac ultrasonography was performed for 10 s only. We suggest that cardiac ultrasonography should be performe… Show more

“…This study also challenges the previous guidance that termination of resuscitation decisions can reliably be based on the finding of absence of electrical activity, or asystole. [ 35 ] There have now been reported cases of successful resuscitation from asystole, [ 31 , 36 ] and with the technical capability evolving, it may be possible in the future to salvage a proportion of these patients.…”

“…One study used TEE to identify intracardiac thrombus [ 28 ], and another one looked into the impact of prehospital echocardiography on treatment decisions [ 29 ]. We also excluded two abstracts with insufficient data for analysis [ 30 , 31 ], and a case report [ 32 ].

“…In the present study, interestingly almost all survivors presented with asystole on arrival of the EMS team. There are only a few reported cases of return of spontaneous circulation after asystole in traumatic CA 13. One possible explanation for this discordance could lie in the origin of the TCA.…”
