Predictive Validity of a Questionnaire That Identifies Older Persons at Risk for Hospital Admission

Abstract: The instrument, which was previously found to be valid in a national sample of Medicare beneficiaries, appears to be valid also in a local sample of Medicaid beneficiaries. Older adults at risk of heavy hospital use can be identified prospectively through their responses to this brief, mailed, self-administered questionnaire. The instrument may be useful in targeting older persons for interventions designed to prevent the need for hospital care.

“…Eles concluíram que, por meio dessas oito variáveis, seria possível a identificação dos idosos com alto risco de serem hospitalizados duas ou mais vezes, em um período de quatro anos. Posteriormente, Boult et al 9 verificaram a confiabilidade em teste-reteste desse instrumento, e Pacala et al 10 estudaram a sua validade preditiva. No Brasil, foi utilizado por Veras et al 8 , com idosos em um guichê de recepção de um serviço de saúde público.…”

“…Based on these results, Boult et al 1 developed a Probability of Repeated Hospital Admissions (Predição de Risco de Admissão Hospitalar -PRA) questionnaire. 1,3,12,13 In accordance with the presence or not of risk factors, the logistic regression model applied to PRA generates a value from 0 to 1. Higher values correspond to increased risk of hospital admittance.…”

“…In contrast, older people identified as high risk by the fourth survey method have been shown not only to generate high healthcare expenditures in the future, [8][9][10] but also to have high levels of chronic disease and functional disability, 11 making them good candidates for case management or disease management. This survey, the "Probability of Repeated Admission" (PRA), collects and analyzes information on eight risk factors to identify older people at high risk for repeated admission to acute care hospitals.…”
