Predictors of Alcohol Misuse and Abuse in Older Women

Abstract: The "best predictor model" showed that older women who were at risk for alcohol misuse or abuse had T-ACE scores of 1 or higher, used two or more OTC drugs regularly, drank large amounts of coffee, used alcohol to fall asleep, and had less sleep latency. Because positive T-ACE scores have high sensitivity and specificity for alcohol abuse, scores of 1 or greater should be addressed in clinical settings, e.g., referrals for more definitive diagnoses and relevant treatment.

“…In women older than age 60, using alcohol to sleep and shorter sleep onset latency each are associated with greater risk for alcohol misuse. 42 However, moderate alcohol use is associated with fewer insomnia symptoms in women, but not in men, older than age 65. 43 In a study of healthy men and women, selfreported insomnia symptoms at baseline were associated with greater odds of heavy drinking at a 5-year follow-up.…”

“…With a much lower cutoff score, the T-ACE was found to be a valid predictor in older women. Stevenson and Masters (2007) compared the T-ACE, a variety of biomarkers, and behavioral and physical measures to determine the best predictor of the quantity and frequency of alcohol use in female drinkers ( n = 72) and abstainers ( n = 63) with a mean age of 70 years. They found that any score greater than zero on the T-ACE was the best predictor of alcohol use quantity and frequency.…”

“…These questions were considered standard at the time and were based on the NSDUH questionnaire. Health Cheq also collected the standard Cut down, Annoyed, Guilty, and Eye-opener (CAGE) questionnaire for alcohol if male 26 or the Tolerance, Annoyance, Cut down, and Eye-opener (TACE) questionnaire if female, 27 as well as the CAGE Adapted to Include Drugs (CAGE-AID) for all illicit drugs. 28 Research assistants then screened patients in person using two prescription drug misuse screening instruments: a) prescription medication CAGE (RxCAGE), a four-item screener, using CAGE alcohol use disorder items modified to focus on prescription drugs 26 and b) Prescription Opioid Misuse Index (POMI-e), expanded from the original focus on prescription opioid misuse to include all prescription drugs.…”
