Predictors of Clostridium difficile infection and predictive impact of probiotic use in a diverse hospital-wide cohort

Abstract: Emergency department location, high-risk antibiotics, probiotics, and statins were independently predictive of CDI. Further exploration of the relationship between probiotics and CDI, especially in diverse patient populations, is warranted.

“…In contrast to the above trials, Carvour et al found that statin use may be a predictor for CDI development among hospitalized patients in New Mexico. 36 This study found that probiotic use was associated with CDI development, also contrasting with data from prior studies. 22 Although multivariate regression was used, other confounders may include statin use as a measure of patients more likely to utilize the health-care system or to present to the hospital with symptoms.…”

“…Studies that did find a significant positive effect of probiotics in these conditions were associated with a rarely encountered C. difficile infection rate of over 5%, [61][62][63] and one trial even associated probiotics use with enhanced incidence of C. difficile infections. 64 These conflicting results greatly highlight a need for high-quality, sufficiently powered prospective trials to resolve the usefulness of probiotics in this and other indications. Conflicting findings were also reported in the context of irritable bowel syndrome (IBS), [65][66][67] respiratory infections, 12,[68][69][70][71][72][73][74][75][76][77][78][79][80][81][82][83][84] Helicobacter pylori infections, [85][86][87][88] inflammatory bowel disease, 89 improving antiretroviral treatment, 90,91 and the metabolic syndrome.…”
