Predictors of fatigue following the onset of infectious mononucleosis

Abstract: Background. Infectious mononucleosis (IM) is a risk factor for chronic fatigue. Reduced activity is the most consistent factor found to be associated with poor outcome following the onset of infectious mononucleosis. However, little is known about the biological mechanisms involved in the pathogenesis of chronic fatigue following IM and no study, so far, has examined the relation between certain illness beliefs and poor outcome. This study explored immunological, endocrine, behavioural and cognitive responses … Show more

“…Studies focusing on fatigue in specific groups at risk found no predictive role of cortisol in the development of post-infectious unexplained fatigue in 71 primary care patients after 3 or 6 months [41], or in postoperative unexplained fatigue in 161 patients undergoing elective surgery after two days, three weeks, or six months [42]. From a study on fatigue in the general population, however, another picture emerges.…”

“…The literature gives ample evidence of chronic fatigue syndromes after infections with viruses like Epstein-Barr virus [1], Ross River virus [2], Coxiella burnetii [3,4], and enterovirus [5,6]. However, also noninfectious diseases have been associated with chronic fatigue such as rheumatic arthritis [7], cancer [8,9], or Crohn's disease [10].…”

“…The finding that the more pronounced the HPA axis changes, the longer the CFS, supports the latter assertion [30]. Furthermore, two prospective studies of groups at high risk of developing chronic fatigue (post-EBV infection and post-surgery) found that the development of fatigue 6 months after was not associated with any HPA axis changes [31,32]. However, one study [33] suggested the opposite, showing that a subset of healthy regular exercisers developed pain and fatigue after being experimentally deprived of exercise for 1 week, and these individuals had lower HPA axis function at the baseline of the experiment.…”
