Predictors of Heart Failure Self-care in Patients Who Screened Positive for Mild Cognitive Impairment

Abstract: In this sample, patients who screened positive for MCI, on average, had adequate HF knowledge yet inadequate self-care scores. These models show the influence of modifiable and nonmodifiable predictors for patients who screened positive for MCI across the domains of self-care. Health professionals should consider screening for MCI and identifying interventions that address HF knowledge and social support. Further research is needed to explain the racial differences in self-care.

“…19 These deficits can compromise the ability of HF patients to recognize and respond appropriately to worsening symptoms. 23 Studies have shown that HF patients with CI struggle more with medication management, are less likely to seek outpatient treatment, and fail to recognize worsening symptoms.…”

“…Assessment of cognitive function is especially important in this group as postoperative management of medications, device controllers, and self-care by the patient is vital to long term survival. 19 The effects of long term hemodynamic support with LVADs on cognitive function are poorly investigated. Two studies using trial patients from the HeartMate II DT and BTT trials determined that, compared to 1 month post-implantation, cognitive function improved at 24 months and 6 months postimplantation, respectively.…”

“…In turn, MCI increases risk of dementia with an annual conversion rate of 10–15% [3]. Cognitive impairment impacts adequate self-care [4], predicts repeat hospitalizations, and is associated with a 5-fold increase in mortality rate [5, 6]. The Montreal Cognitive Assessment (MoCA) is commonly used in research as a multicategorical measure of cognitive function and is more sensitive than other measures such as the Mini-Mental State Examination [7].…”
