Predictors of major vascular events in patients with a transient ischemic attack or nondisabling stroke. The Dutch TIA Trial Study Group.

Abstract: The Dutch TIA Trial Study Group Background and Purpose: Very few studies have addressed the identification of prognostic factors in patients with a transient ischemic attack or minor stroke. Methods: We prospectively studied the prognostic value of clinical, laboratory, radiological, and electrocardiographic findings in 997 patients with a transient ischemic attack and 2,130 patients with a minor stroke who were entered into a multicenter clinical trial. The mean follow-up period was 2.6 years. Results: A total … Show more

“…As in other studies, the incidence of retinal infarctions was low [19, 23, 25, 27]. Overall, patients with TMB associated with atheromatous disease have a lower risk of ipsilateral stroke than patients with cerebral TIA [5, 6]. In contrast, we found that the risk of cardiac disease in patients with TMB was not lower than that reported in studies on cerebral TIA [23, 28].…”

“…Upward resolution of visual field loss might occur when restriction of blood flow to the lower part of the retina is restored last. In other cohorts of patients with TMB or TIA in general, the occurrence of multiple attacks has also been found to predict recurrent vascular events [4, 6]. Involvement of the peripheral, but not the central area of the visual field was identified as the most powerful predictor of impending vascular disease in our study.…”

“…In the Dutch TIA Trial, 997 patients with TIA and 2,130 patients with minor stroke were enrolled for 2 year risk analysis for outcomes of fatal or non-fatal stroke in addition to myocardial infarction, stroke, or vascular death (7). It identified the following independent risk factors for stroke, myocardial infarction or vascular death: age > 65 years; male sex; dysarthria; multiple attacks; diabetes; angina pectoris; intermittent claudication; computed tomographic evidence of any cerebral infarct especially a border zone infarct or white matter hypodensity; and electrocardiographic evidence of anteroseptal infarct, ST depression, left ventricular hypertrophy, or left atrial conduction delay.…”
