Predictors of Non-Sentinel Lymph Node Metastasis in Patients with Positive Sentinel Lymph Node in Early-Stage Cervical Cancer: A SENTICOL GROUP Study

Basile Pache,
Matteo Tantari,
Benedetta Guani
et al.

Abstract: Background: The goal of this study was to identify the risk factors for metastasis in the remaining non-sentinel lymph nodes (SLN) in the case of positive SLN in early-stage cervical cancer. Methods: An ancillary analysis of two prospective multicentric databases on SLN biopsy for cervical cancer (SENTICOL I and II) was performed. Patients with early-stage cervical cancer (FIGO 2018 IA to IIA1), with bilateral SLN detection and at least one positive SLN after ultrastaging, were included. Results: 405 patients … Show more

“…However, no specific analysis was performed for patients with LVM confined to the SLN only. Recently, Pache et al [ 34 ] published an ancillary analysis of SENTICOL 1-2 trials to exploit the risk factors associated with non-SLN metastases in patients with positive SLN. All non-SLNs were analyzed via ultrastaging to avoid missing LVM.…”

“…Comprehensive regional nodal irradiation is recommended for patients with LVI according to the NCCN guidelines 9 . Furthermore, the presence of LVI is related to non-sentinel lymph node involvement in patients with a positive sentinel node biopsy 10 . Currently, LVI status is determined using surgical pathology specimens, which can lead to complications such as delayed treatment, additional surgeries, unnecessary anxiety, and diminished quality of life.…”
