Predictors of Recurrence After Local Excision and Postoperative Chemoradiation Therapy of Adenocarcinoma of the Rectum

Abstract: Local excision and postoperative radiotherapy result in adequate local control of early stage (Tis and T1) adenocarcinoma of the rectum. Higher rates of recurrence were seen in patients with T2 and T3 tumors, especially in those with positive margins.

“…In single-arm studies, especially in T2 stage I rectal cancers, high rate of local recurrence, i.e. 28%, was detected, despite the administration of post-operative chemoradiotherapy (Ota et al, 1992;Bleday et al, 1997;Bouvet et al, 1999;Chakravarti et al, 1999).…”

“…Recent studies reported the incidence of local failure as 0-5% for T1, 10-20% for T2, and 24-50% for T3 lesions [17,18]. In these studies, recurrence was also more likely to occur in patients with positive margins than in those with negative margins.…”

“…T1 tumors have a 6-12% incidence of lymph node (LN) involvement, T2 tumors have positive lymph nodes (LNs) in 17-22% of the cases and T3 tumors have LN involvement in m o r e t h a n 6 6 % o f t h e c a s e s . T h e aforementioned differences are reported to result in different 5-year local recurrence rates, namely 5% for T1 tumors, 18% for T2 tumors and 22-33% for T3 tumors (Bouvet et al, 1999 www.intechopen.com…”
