Predictors of Successful Extubation in Children

Abstract: Objective criteria to predict extubation outcome in mechanically ventilated children are not available. Our goal was to study factors associated with extubation success and to evaluate the usefulness of the rapid shallow breathing index (RSBI) and the compliance, resistance, oxygenation, and pressure index (CROP index) in children. Data were prospectively collected on 227 mechanically ventilated children. Patients successfully extubated had significantly better lung compliance (Cdyn: 0.59 +/- 0.91 versus 0.39 … Show more

“…Given the wide variability in each child's response to intubation and the multitude of risk factors involved (e.g., history of traumatic or repeated intubation, size of ETT, prematurity, presence of concurrent gastroesophageal reflux) (38)(39)(40)(41), a generalized clinical model of acquired SGS may not be applicable to all intubated neonates. Hence, serial imaging of each airway would allow clinicians to monitor for edema or fibrosis and make individualized airway management …”

“…For example, the Rapid Shallow Breathing Index (RSBI) is usually defined as RSBI = RR/V T , where RR is the respiratory rate and V T refers to tidal volume. By the original definition of RSBI, V T and RR are measured without ventilator support [10]. In MIMIC-II, V T and RR are always measured with active ventilator support (at least for our patient population of intubated neonates), so it should be noted that our calculation is not as accurate, but is still a metric of patient respiratory ability.…”
