Pregnancy and Melanoma: Recommendations for Clinical Scenarios

Abstract: Managing pregnant patients with a history of melanoma or with a melanoma diagnosis can be daunting and confusing for dermatologists. We present three clinical scenarios that raise questions about the safety of pregnancy in patients with a history of melanoma, skin biopsies during pregnancy, and excisions and sentinel lymph node biopsies during pregnancy. Our recommendations incorporate the most up-to-date clinical data to help guide clinicians when faced with pigmented lesions and melanoma in a pregnant patien… Show more

“…Despite immunologic changes during pregnancy that may decrease immune surveillance and the finding that some melanomas harbor sex-hormone receptors, the preponderance of evidence does not support a poorer prognosis for pregnancy-associated melanomas. 15 Ovarian stimulation was not found to induce changes in moles, and pregnancy was similarly not associated with a difference in melanoma thickness. 14,16 Fertility preservation for oocyte or embryo cryopreservation, however, does introduce a delay of 2-5 weeks to the start of anticancer therapy and may not be applicable to those who require immediate treatment.…”

“…82 Pregnancy-associated melanoma metastases were found in 30 cases of placental metastases, whereas only 8 such cases have been reported in the literature. 35,37,45,82,86 Within the first year of life, the disease led to death in 6 of them. It is suspected that such a small number of described patients are associated with insufficient studies, because the majority of placentas are not subjected to histopathological examination, and moreover, the observation of children of mothers with MM was too short.…”
