An experimental model of massive small bowel resection and colonic autograft in an antiperistaltic fashion in between the remnants of jejunum and ileum was developed in Rhesus monkeys. The gross morphological changes showed significant (p < 0.001) lengthening and dilatation of the remnant jejunum, ileum and the colonic graft. Hypertrophy and hyperplasia of the mucosal and muscular layers of all the 3 segments was revealed by morphometric studies and the estimation of the nucleic acid content of the mucosa. All the 3 segments of the bowel showed a significant increase (p < 0.001) in the uptake of D-glucose, L-alanine and L-phenylalanine when compared to the control animals. These changes were more pronounced in the animals kept for longer duration. Phlorizin was found to be inhibitory for the uptake of D-glucose in the grafted colon, suggesting the induction of an active nonelectrolyte transport process in the autografted colon. The present study suggests that the colonic loop adapts itself to the functions of the small bowel along with the compensatory adaptation of the remnant jejunum and the ileum for the loss of absorptive surface secondary to massive small bowel resection.

Short bowel syndrome is a clinical entity that results from a diverse group of congenital and acquired conditions in the pediatric population. The pathophysiology of this syndrome is characterized by malabsorption, malnutrition, and metabolic disturbances. The vast majority of children with this condition undergo spontaneous adaptation of the intestinal remnant and achieve enteral nutritional autonomy. However, a small portion of pediatric patients develop intestinal failure and require long-term or permanent dependence on total parenteral nutrition. These children may benefit from surgical interventions that facilitate intestinal adaptation. Such adjunctive procedures facilitate nutrient absorption by improving motility, prolonging intestinal transit, and/or increasing mucosal contact time. In selected patients, this may allow them to be weaned from parenteral nutritional support or to have it discontinued. The purpose of this review is to discuss the various surgical techniques for the management of short bowel syndrome in children, along with their indications, complications, and outcomes.

Öz GirişBağırsak sisteminin bireyin besin alımı ve sıvı dengesini sağlamada yetersiz kaldığı durumlara bağırsak yetmezliği adı verilmektedir. Bağırsak uzunluğunun azaldığı, ileoçekal valvin olmadığı, kolonun kaybedildiği gibi uzunluk kayıpları veya mukozal yetmezlik, dismotilite veya aktif enterit gibi besin emiliminin mümkün olmadığı durumlarda birey kalıcı veya uzun süreli total parenteral beslenmeye mecbur kalabilmektedir (1) . "Büyüme için gerekli sıvı ve besinlerin sindirimi ve emilimi için yeterli miktarda fonksiyonel bağırsak kalmaması" pediatrik bağırsak yetmezliğinin modern tanımlamasıdır. Bir başka tanımda ise, büyüme, hidrasyon ve elektrolit homeostazının bozulmasına neden olan bağırsak hastalığı olarak genellenebilmektedir.
