Objective: To evaluate the diagnostic accuracy of fluorine-18 fluorodeoxyglucose positron emission tomography (¹⁸F-FDG PET) in the detection of bladder lesions. Methods: We conducted a systematic PubMed/MEDLINE and Embase database search of articles published before November 2012. Sensitivity, specificity, likelihood ratio and diagnostic odds ratio were pooled. A summary receiver operating characteristic curve was also used to summarize overall test performance. All meta-analyses were performed using the Meta-DiSc software (version 1.4). Results: Six studies met the inclusion criteria. The pooled sensitivity and specificity of PET or PET/CT for the detection of bladder cancer was 80.0% (95% CI: 71.0-87.0%) and 84.0% (95% CI: 69.0-93.0%), respectively. The overall positive likelihood ratio, negative likelihood ratio and diagnostic odds ratio were 3.47 (95% CI: 1.03-11.65), 0.31 (95% CI: 0.13-0.70) and 13.86 (95% CI: 2.84-67.74), respectively. Besides, the area (± standard error) under the symmetrical summary receiver operating characteristic curve was 0.8574 ± 0.0704. Conclusion: When compared with results of MRI and CT published by other studies, ¹⁸F-FDG PET or PET/CT showed no superiority in detecting local bladder lesions. As a whole body imaging, it is suggested that PET is more appropriate for the detection of metastasis.

ObjectivesWe aim to assess the clinical value of 18F‐fluorodeoxyglucose positron (18F‐FDG‐PET) scan in detecting nodal and distant metastasis compared with computed tomography (CT) scan in patients with urothelial carcinoma or bladder cancer, aiming to improve staging accuracy and thereby better prognosticate and determine therapy.MethodsA retrospective review of 75 patients with invasive bladder cancer (≥T1) who were staged with both CT and 18F‐FDG‐PET within an 8‐week interval was performed for the period between 2015 and 2020. Seventy‐two per cent (54/75) had formal pelvic lymph node (LN) dissection or biopsy of lesions suspicious for metastases. FDG‐PET definitions for positive sites were assessed depending on SUV Max (nodes with SUVmax >4 at any size, SUV > 2 for lymph nodes >8 mm, or any SUV if the lymph node was >10 mm on axial images). For CT scanning, enlarged LN by RECIST 1.1 criteria (>10 mm) as well as qualitative findings suggesting metastasis were considered positive. The analysis was based on the comparison of CT and 18F‐FDG‐PET findings to histopathology results from LN dissection or biopsies.ResultsSensitivity, specificity, positive predictive values (PPV) and negative predictive value (NPV) of CT versus FDG‐PET for detecting metastasis, in patients who underwent pelvic LN dissection or biopsy of lesions suspicious of metastases, were 46.6% (95% CI: 21%–70%) versus 60% (95% CI: 32%–84%), 100% (95% CI: 91%–100%) versus 83.78% (95% CI: 69%–94%), 100% (95% CI: 63%–100%) versus 60% (95% CI: 32%–84%), and 82.2% (95% CI: 68%–92%) versus 83.78% (95% CI: 69%–94%), respectively. 7/75 (9.3%) patients avoided cystectomy due to 18F‐FDG‐PET features of metastases that were not detected by CT.ConclusionFDG‐PET may be more sensitive than CT for metastases in the staging of bladder cancer, which resulted in significant avoidance of aggressive local management in cases with occult metastasis.

Positron emission tomography (PET) using ((18)F)2-fluoro-D-2-desoxyglucose (FDG) has been shown to be a highly sensitive and specific imaging modality in the diagnosis of primary and recurrent tumors and in the control of therapies in numerous non-urologic cancers. It was the aim of this review to validate the significance of PET as a diagnostic tool in malignant tumors of the urogenital tract. A systematic review of the current literature concerning the role of PET for malignant tumors of the kidney, testicles, prostate, and bladder was carried out. The role of FDG PET for renal cell cancer can be seen in the detection of recurrences after definitive local therapy and metastases. The higher sensitivity of PET in comparison to other therapeutic modalities (CT, ultrasound, MRI) in recurrent and metastatic renal cell cancer suggests a supplemental role of this diagnostic procedure to complement other imaging modalities.The clinical value of PET is established for the identification of vital tumor tissue after chemotherapy of seminomatous germ cell tumors. This diagnostic method has little significance for primary tumor staging and diagnosis of non-seminomatous germ cell tumor because of the high probability of false-negative results in adult teratomas. FDG PET is not sensitive enough in the diagnosis of primary or recurrent tumors in prostate or bladder cancer. Also PET did not prove to be superior to conventional bone scintigram in the detection of mostly osteoblastic metastases in prostate cancer. The recent use of alternative tracers, which are partly not eliminated by urinary secretion (acetate, choline) has increased the sensitivity and specificity of PET also in this tumor entity so that further clinical investigations are needed to validate these technical modifications in their significance for this imaging modality. PET appears to be sufficiently evaluated only for the diagnostic follow-up of patients with seminomatous germ cell tumors after chemotherapy to regard it is the diagnostic tool of first choice. For all other tumors of the urogenital tract this proof is still awaited.
