Preliminary studies of the ICD-11 classification of personality disorder in practice

Abstract: The ICD-11 classification of personality disorder yields somewhat higher levels of personality dysfunction than ICD-10, possibly because the age range for the onset of diagnosis is now flexible. The range of severity levels make the classification more useful than ICD-10 in clinical practice as it identifies the greater pathology necessary for intervention.

“…It has been proposed that the upcoming ICD-11 diagnostic criteria would utilize a more dimensional approach to personality disorders and thus, enable clinicians to take better into account the dimensional nature of personality and reduce the problem associated with the overlap of the different disorder categories [8, 9, 20, 36] The currently suggested personality disorder levels are: mild personality disorder, moderate personality disorder, and severe personality disorder [1]. “Mild personality disorder” is suggested to be used as an additional entity to denote characteristics that significantly influence a person’s health status and the use of health services, which are in general classified as Z codes under the current ICD-10 [37].…”

“…In contrast, there is a vast body of empirical literature supporting dimensional models of personality disorder that are closely aligned with the proposed model , in addition to the emerging body of work on the specific dimensions proposed for ICD‐11 . The ICD‐11 proposal has two elements.…”

“…It remains to be seen if the new domain structure is more robust and likely to be superior. One study suggests it has better links than ICD‐10 did (Tyrer et al, ). Other studies are comparing the domain structure of ICD‐11 with the operational criteria for personality disorder in the DSM‐IV, and these will also inform.…”
