Premenstrual syndromes: closing the 20th century chapters

Abstract: The need to re-evaluate premenstrual syndromes became apparent in 1997-1998 and early 1999. The success stories of some symptomatic treatment modalities and more sophisticated studies of pathobiology chart the pathways for future progress: the shift from a descriptive diagnosis to diagnoses based on etiology, the recognition of diversified vulnerabilities and their expression in particular situations, and specific treatment modalities.

“…Although the PMDD diagnosis remains controversial, its provision of systematic diagnostic criteria has contributed to considerable gains in identifying effective symptomatic treatment modalities and investigating the pathobiology of the disorder. Several reviews summarize these developments, which have occurred primarily over the past decade [2][3][4][5][6][7]. The purpose of this review is to describe the current status of diagnosis and treatment of PMDD with psychotropic and hormonal medications, with a focus on reports that have been published in the medical literature during the 2001 to 2002 interval.…”

“…plays an important role in the pathogenesis of PMS (Lepage and Steiner 1991;Rapkin 1992;Halbreich 1999). The cumulative effects of oestrogen act as an agonist on serotonergic (5-HT) function by increasing the number of serotonin receptors, serotonin postsynaptic responsiveness, neurotransmitter transport and uptake (Halbreich et al 1995;Matsumoto et al1985;McEwen et al 1997).…”

“…Psychiatric researchers now hypothesize that premenstrual changes may result from a temporary impairment of homeostasis among a multitude of intrapersonal and environmental systems (Halbreich, 1995;Halbreich, 1999). This impairment can be triggered by a differential pace and magnitude of changeover-time in levels of several hormones and other substances during the luteal phase.…”
