Prenatal Counseling and Care for Single-Ventricle Heart Disease: One Center’s Model for Care

Abstract: Advances in prenatal imaging allow early detection of single-ventricle congenital heart disease, which may enhance prenatal care and maximize care options and decision making. Boston Children's Hospital's Advanced Fetal Care Center and fetal cardiology program provide prenatal counseling and care for single-ventricle congenital heart disease. Key points for optimal prenatal counseling and education include explanation of the diagnosis, delivery, the first surgery, cardiac neurodevelopmental issues, feeding and… Show more

“…In this review, both the nursing and medical literature agreed that nursing psychosocial support and counseling are essential in fetal cardiology care. With a diagnosis of fetal CHD comes an enormous amount of medically and anatomically complex information, delivered to families at an emotional, stressful time (Lafranchi & Lincoln, 2015;Lee, 2017;Reid & Gaskin, 2018). Nurses are well positioned to give families gentle, straightforward, understandable explanations, as well as answer follow-up questions and provide education throughout the pregnancy (Dopson et al, 2022;Lafranchi & Lincoln, 2015;Lee, 2017;Loyet et al, 2016;Nagata et al, 2022).…”

“…Patients have reported relief and support from nurses after receiving the diagnoses of CHD in their fetus (Bratt et al, 2015;Reid & Gaskin, 2018). Some articles also stressed the importance of continuous, on-going support provided by nurses throughout the patient's pregnancy and beyond (Bratt et al, 2015;Kovacevic et al, 2020b;Kovacevic et al, 2021;Lafranchi & Lincoln, 2015;Lee, 2017). The nurse's presence from preconception to birth allows excellent continuity of care for the patient and family.…”

“…The second category is the type of CHD that causes too little blood to pass through the lungs. This category includes six types: (a) Tricuspid Atresia, a lack of formation of the tricuspid valve and a lack of a direct connection between the right atrium and right ventricle; (b) Pulmonary Atresia, a defect in the development of the pulmonary valve; (c) Transposition of the Great Arteries (TGA), a defect in which the location of the pulmonary artery and the aorta are reversed; (d) Tetralogy of Fallot (TOF), a defect that consists of four problems, including a whole between the lower chambers of the heart; (e) Double outlet right ventricle, a condition where both the aorta and the pulmonary artery are connected to the right ventricle; and (f) truncus arteriosus, which occurs when a single great ventricle does not separate completely [15].…”

“…Another defect is the bicuspid aortic valve (BAV), which is an aortic valve with only two leaflets that should have three. This valve regulates blood flow from the heart to the aorta as well as the major blood vessels, which brings blood to the body [15].…”
