Prenatal Diagnosis of a Patent Urachus Cyst with the Use of 2D, 3D, 4D Ultrasound and Fetal Magnetic Resonance Imaging

Abstract: Patent urachus cyst is a rare umbilical anomaly, which is poorly detected prenatally and frequently confounded with pseudo bladder exstrophy or omphalocele. A 27-year-old woman was referred to our prenatal diagnosis centre at 18 weeks of gestation after diagnosis of a megabladder and 2 umbilical cord cysts. Subsequent 2D, 3D and 4D ultrasound examinations and fetal magnetic resonance imaging (MRI) revealed a typical umbilical cyst and an extra-abdominal cyst, communicating with the vertex of the fetal bladder … Show more

“…However, pseudoexstrophy of the bladder has specific findings on prenatal ultrasonography. First, a cystic mass has a wide communication with the bladder, whereas a patent urachus has a narrow communication . Second, a mass lies below the two umbilical arteries, whereas a patent urachus is centered between the umbilical arteries .…”

“…First, a cystic mass has a wide communication with the bladder, whereas a patent urachus has a narrow communication . Second, a mass lies below the two umbilical arteries, whereas a patent urachus is centered between the umbilical arteries . Third, a mass persists as gestation progress, whereas a patent urachus disappears later in gestation, probably rupturing as urine output increases at gestational weeks 22 ~ 32 .…”

“…Indeed, defects in the lower abdominal wall, as well as abnormal traction of the umbilical cord, seen in omphalocele, were most likely to have a deleterious effect on normal development from the urogenital sinus to the bladder [1011]. Recent advances in ultrasound techniques revealed urachus defects in fetuses in utero [1213]. However, abnormalities in the vas deferens and/or colliculus seminalis, even if present, would be found after puberty.…”

“…Allantois is a rudimentary embryological structure placed centrally in the umbilical cord 1 3. Allantoid cysts have previously been diagnosed with three-dimensional ultrasound and magnetic tomographic imaging and the relationship between the cysts and vessels can be visulaised 2 9. Allantoid cysts separate the umbilical cord vessels, while pseudocysts and omphalomesenteric cysts displace the vessels 2 5 6 8.…”
