Prenatal ultrasonic findings in congenital chloride diarrhoea

Abstract: In 3 fetuses affected by congenital chloride diarrhoea there were characteristic prenatal ultrasonic findings. In all cases the fetal abdominal cavity was filled with distended loops of fetal intestine, the fetal stomach was of normal size and polyhydramnios was present. The prenatal ultrasonic findings in this metabolic inherited disease are identical to those caused by mechanical obstruction of the distal fetal intestine. Whenever these findings are encountered prenatally, the possibility of chloride diarrho… Show more

“…Fetal abdominal distension (due to fluid accumulating in their intestine)7 can be considered pathognomonic for CCD, however dilated bowel loops could be present in other diseases like cystic fibrosis or intestinal obstruction. However, in cystic fibrosis, the dilated bowel loops are hyperechoic due to the viscosity of meconium11 and in intestinal obstruction dilated intestinal loops is associated with increased peristalsis in few dilated bowel loops 12…”

“…Most children with CCD are born preterm and their antenatal ultrasound usually reveal polyhydramnios with significant dilatation of the fetal intestinal loops 7. Postnatally, these children exhibit significant abdominal distension, profuse and watery stools, and failure to thrive.…”

“…Patients are born slightly prematurely because of intrauterine diarrhea [9]. After birth, the persistent intestinal loss of acidic fluid and chloride leads to hypochloremia, metabolic alkalosis, hyponatremia, hypokalemia, and dehydration.…”

“…Chronic hypovolaemia and hypokalaemia will result in renal tubular dysfunction in some of these children. Children with CCD are at increased risk of developing volvulus postnatally, a complication that may occur at any age from 2 days to 15 years 6. If the diagnosis is made early and treatment is given adequately, children with CCD can achieve normal physical growth and mental development.…”
