Preoperative Chemotherapy for Operable Breast Cancer Is Associated with Better Compliance with Adjuvant Therapy in Matched Stage II and IIIA Patients

Abstract: Introduction. Preoperative chemotherapy (PC) for operable breast cancer has shown significant benefits in prospective trials. Many patients are treated in the community setting and some may question the applicability of PC outside the university setting.Methods. Retrospective review was performed of stage II and IIIA breast cancer patients treated from January 2002 to July 2009. Fifty-three of 57 patients who underwent PC were matched based on age, tumor size, and hormone receptor status with 53 patients who d… Show more

“…The possible mechanism is that the oestrogen level may affect the sensitivity of tumour cells to chemotherapy [14]. Furthermore, it is well known that ER+/PR-tumours have a higher level of growth signalling than ER+/PR + tumours [15,16]. However, in our study, there was no significant difference when analysing the response to NACT among various ER and PR status subgroups (ER+/PR + and ER+/PR-subgroups).…”

“…Another study found that smokers were more likely to be nonadherent. Komenaka and colleagues suggested considering current smoking as a proxy marker for a certain personality type unwilling to adhere to medical guidelines (28). While a low BMI is associated with a high socioeconomic status in the Western world (37,39), its inclusion in the clinical profiles increased the profiles' predictive accuracy.…”

“…Studies have identified, with often conflicting results, that factors associated with poor adherence to breast cancer therapy such as type of breast cancer surgery (16,17), severity of side effects (18,19), number of tablets (20)(21)(22), age (11-13, 15, 20, 23, 24), depression and/or the use of antidepressants (24,25), socioeconomic disadvantage (26,27), smoking (28), the impact of breast cancer therapy on quality of life, and the perceived riskbenefit ratio (29).…”

“…o volume de tecido removido também foi menor no grupo com quimioterapia neoadjuvante (143,6 cm 3 contra 273,9 cm 3 , p=0,003) 8 . esses autores também demonstraram que a aderência ("compliance") ao tratamento foi maior no grupo tratado com quimioterapia neoadjuvante versus adjuvante (100% versus 70%, p=0,0001) 9 .…”
