Preoperative hemostasis and its association with bleeding and blood component transfusion requirements in cardiopulmonary bypass surgery

Abstract: A standardized bleeding history may help to identify patients undergoing CPB surgery whose risk of bleeding is elevated. ADP-induced MEA appears to predict postoperative bleeding and PC transfusion requirements, while AA-induced MEA and preoperative Hb indicate the need for RBCs. The time of aspirin withdrawal before surgery influences perioperative blood loss and PC transfusion.

“…These findings independently confirm the findings of the first study 3 and other studies. 20,[68][69][70][71][72] In a systematic review, lower Hb and older age were identified as independent predictors of transfusion in orthopedic procedures. 73 Other factors commonly found to be associated with increased risk of transfusion included complexity of surgery, low body weight, presence of comorbidities, and female sex.…”

“…Aspirin and warfarin could affect to the same extent on perioperative bleeding (Sam et al , ). However, preoperative aspirin discontinuation duration had associated with perioperative bleeding in cardiac surgery (Emeklibas et al , ). Longer duration of aspirin and clopidogrel discontinuation in patient not on warfarin therapy than those on warfarin therapy might have slightly lesser influence on perioperative bleeding.…”

“…There was no significant correlation between blood loss and postoperative platelet aggregability. Some previous studies, conducted in cardiac surgery patients, have also reported an association between pre-operative ADP-test and blood loss or requirement for platelet concentrates [21][22][23]. Most of the patients in the previous studies were taking aspirin [22] or thienopyridines [21] at the time of the study, or had stopped in the few days previously, which would influence the correlation between platelet aggregation and blood loss.…”
