Preoperative Pain and Function: Profiles of Patients Selected for Total Knee Arthroplasty

Abstract: Background Total knee replacement (TKR) is an effective treatment to relieve pain and restore function in patients with advanced knee osteoarthritis (OA). TKR utilization is growing rapidly, and the appropriateness of current TKR use is of great interest. We examined patient-reported pre-operative pain and function profiles to understand symptom severity at the time of TKR decision. Methods Data were from FORCE-TJR. We included patients undergoing primary, unilateral TKRs between 2011 and 2014 for OA, and ha… Show more

“…Indeed, these numbers may help clinicians consider their patients’ physical ability on a larger scale and compare them according to their knee conditions, while keeping in mind that participants without knee condition also reported physical limitation (figure 1). Disparities were observed regardless of age and are consistent with other studies 10 11. We observed other associations, which are known to be associated with knee osteoarthritis: in the knee condition groups, there is a higher number of obese people, fewer people working and more people who had no secondary education 12…”

“…As mentioned above, by the time the majority of arthroplasty patients reach the operating theatre doors, they will have dealt with increasing pain and decreasing mobility and function. This is particularly true for the TKA patient (Nguyen, Ayers, Li, Harrold, & Franklin, ). In both the findings of this study and in prior PPI work, patients discussed pain in the context of both preoperative and postoperative experiences.…”

“…Evaluating the minimal clinically important difference, and substantial clinical benefit of different treatments for hip and knee OA for different population profiles is one such opportunity 61 . This technology may also provide great predictive insights into the performance of organizations, surgeons, and multi‐disciplinary teams alongside equitable patient‐specific payment models 62 . AI‐enabled solutions are also evolving and learning systems.…”
