Preoperative Ultrasound-Guided Wire Localization of the Lateral Femoral Cutaneous Nerve

Abstract: Preoperative USWL is a useful technique that minimizes the time needed to find the LFCN. For the less experienced surgeon, it is extremely valuable. For the experienced surgeon, it can identify anatomical abnormalities such as duplicate nerves, which may not be readily recognizable without ultrasound. Collaboration between the surgeon and the radiologist is very important, especially in the early cases.

“…Although some surgeons may perform and interpret their own US, it can demand valuable operative time, and the US machines typically found in the operating room are not as advanced as those in the radiology department. 15 In our patient, the minimally invasive transtubular approach associated with a CT ACS marking allowed great precision to locate the INS under the rib, its gross total resection, and pleural open avoidance. To our knowledge, this technique has not been previously described.…”

“…Although ultrasound has proven to be increasingly successful for noninvasive peripheral nerve visualization and for assessment of the fan type versus non-fan type of LCFN prior to total hip arthroplasty [11], to our knowledge, preoperative noninvasive ultrasound localization of the LFCN prior to decompressive surgery has not yet been described. Preoperative ultrasound-guided wire localization has been reported with preliminary feasibility; however, the approach is more invasive than using the high-resolution ultrasound alone, as the wire is placed around the nerve and the patient must be under anesthesia [12].…”

“…The patient is placed in the supine position on a stretcher in the radiology suite for ultrasound-guided wire localization of the LFCN. 7 Clear adhesive skin tape is used to hold the patient’s rostrally mobilized abdominal fat pannus in place, allowing for more direct access to the nerve.…”
