The study highlights the joint accreditation program for quality management and good practices in hemotherapy of AABB (American Association of Blood Banks) and ABHH (Brazilian Association of Hematology and Hemotherapy) for blood banks and transfusion services on a national level. The objective of the study was to identify the most prevalent audit findings of Brazilian hemotherapy services in relation to the requirements of the Standard for Blood Banks and Transfusion Services and to analyze the benefits and challenges of the accreditation program. This is a descriptive study, using two complementary phases, which are the analysis of audit reports from the American Association of Blood Banks by using Pareto charts to identify the prevalence of unmet requirements and their respective findings, as well as the application of a semi-structured form to capture information from quality managers regarding the benefits and challenges of the accreditation program. The study showed that both in the accreditation audit and in the reaccreditation audit of the Brazilian institutions that participated in the study, the main requirements of the standard for blood banks and transfusion services that were not met were concentrated in chapters 1 (organization), 5 (process control) or 6 (documents and records) and that in the opinion of the managers of the institutions, the main benefits of the program are: safer processes in relation to donors, patients and professionals on site, optimization of technical processes, and improvement of the quality management system. The main challenges are to adapt existing processes to the specific requirements of AABB, create an institutional culture towards quality processes, comply with the time allotted for the completion of the process and adapt the quality management system.
