Prescribing Analgesics to Older People: A Challenge for GPs

Abstract: Background: Due to population aging, there is an increase in the prevalence of chronic diseases, and in particular musculoskeletal diseases. These trends are associated with an increased demand for prescription analgesics and an increased risk of polypharmacy and adverse medication reactions, which constitutes a challenge, especially for general practitioners (GPs), as the providers who are most responsible for the prescription policy. Objectives: To identify patterns of analgesics prescription for older peopl… Show more

“…By mechanisms of sarcopenia and reduced mobility, these diseases might have contributed to higher levels of frailty in individuals in cluster DFUN ( 79 , 80 ). The close association of musculoskeletal diseases with chronic pain, poor sleep quality, and anxious-depressive disorders, may also explain higher burden of individuals in cluster DFUN with symptoms of mental disorders, compared to individuals in cluster COG-IMP ( 73 , 81 ). The marked expression of symptoms of mental disorders, found in patients in cluster DFUN, may also suggest psychological distraction as a mechanism of the possible importance for cognitive impairments, in patients in this cluster ( 82 ).…”

“…When integrating several pieces of this evidence, there is indication of a close association between chronic pain and mental disorders, anxiety, and depression and that mental disorders usually coexist with musculoskeletal diseases in comorbidity patterns [ 60 , 63 ]. Older persons with chronic pain experience and anxiety and depression use opioid analgesics or psychotropic medications more often than others [ 64 ]. These medications might contribute to the higher expression of functional organ impairments and the development of the physical frailty phenotype in individuals in the PhyF cluster [ 65 ].…”

“…It has been debated whether medical professionals are lacking in courage when it comes to sufficient pharmacological pain treatment in older adults [ 31 , 32 ]. The results herein indicate that analgesics are in substantial use.…”
