Prescribing potentially inappropriate medication (PIM) in Germany's elderly as indicated by the PRISCUS list. An analysis based on regional claims data

Abstract: PRISCUS-PIMs are widely used. Educational programs should focus on drugs with high treatment prevalence and call professionals' attention to those elderly patients who are at special risk for inappropriate medication.

“…We prepared a preliminary PIM list which contained 85 PIM (82 active substances plus one combination of active substances and two different preparations of one substance) from the German PRISCUS list [22] and 99 PIM from the French [3], American [24, 25] and Canadian [26] lists. These tools have been used in research to evaluate the prescription of PIM and factors associated with PIM use [5, 6, 14, 27–29]. The main reason for each drug being PIM was formulated using the information provided by the original lists.…”

“…Numerous studies have been conducted in patients with HTN aged over 65 years [19][20][21][22] regarding antihypertensive treatment adherence and antihypertensive drugs associated with cardiovascular conditions [Study on Cognition and Prognosis in the Elderly (SCOPE), Systolic Hypertension in China (SysTChina), the Coope and Warrender study, Systolic Hypertension in the Elderly Program (SHEP)] [1]. However, little data are available for patients aged 75 years and older regarding key geriatric aspects of fragility (delirium, falls, constipation, etc.)…”

“…Whilst most of these tools have defined elderly as those aged 65 years and above, the IPET and the MAI do not provide any specific definition of 'elderly'. A major limitation of some tools is their potential lack of relevance to other countries (different from their origin), since the listed medications may not be available for use (Schubert et al, 2013). For example, Beers' criteria was modified to generate its country-specific versions, for example, the PRISCUS list-Germany (Holt et al, 2010) and the French consensus panel list-France (Laroche et al, 2007).…”
