Prescribing quality for older people in Norwegian nursing homes and home nursing services using multidose dispensed drugs

Abstract: Sammendrag:Purpose: to examine and compare the quality of drug prescribing for older patients in nursing homes and home nursing services. “…The increased risk of PIMs observed in women is quite surprising. Indeed, gender had no influence on PIMs in any of the 11 studies that studied this relationship [12,41,30,[42][43][44]29,45,46,14,47]. This result may reflect the fact that women live longer, but with a heavier burden of comorbidity, and hence medications.…”

“…Studies that compared drug regimen between institutionalized people and old people living in community have shown higher rates of polypharmacy and PIMs among institutionalized people [11][12][13]. Inappropriate drug use was shown to increase the risk of adverse health outcomes (hospitalizations, emergency department visits, death) in nursing home residents [14][15][16].…”

“…Others also generate data on multi-dose dispensed drugs to assess prescribing quality [21]. These have been used extensively by social pharmacy researchers to gain insights into the rational use of medicines and clinical pharmacists are starting to utilize the possibilities of performing research using these data [22].…”

“…Previous studies in the US and Europe indicate a proportional increase in the number of medications used with age . The use of five or more medications concurrently is common in residential aged care facilities (RACFs) . Growing evidence suggests that polypharmacy in older people is related to inappropriate medications .…”

[5][6][7][8] Growing evidence suggests that polypharmacy in older people is related to inappropriate medications. 9 The proportion of potentially inappropriate medications related to polypharmacy was threefold higher in home health and hospice care patients than found among ambulatory care patients.…”
