Presentation, Management, and Outcome of Thyroglossal Duct Cysts in Adult and Pediatric Populations: A 14-Year Single Center Experience

Abstract: The occurrence of TDC shows a bimodal age distribution. Preoperative evaluation and time for surgery vary whereas clinical presentations, surgical management, and postoperative outcomes are comparable among adult and pediatric groups. Ultrasonography is the preferred diagnostic modality, and the Sistrunk procedure remains the primary choice of treatment.

“…Thus, preoperative imaging, such as sonography, computed tomography, and FNA biopsy are recommended as supplementary techniques to confirm the diagnosis. 15 In our study, we did not identify any patient with ectopic thyroid tissue in the postoperative histopathological examination which other studies demonstrated as commensurate finding. [9][10][11] Complications of the Sistrunk procedure are known to be minor ones and wound-related.…”

“…[9][10][11] Complications of the Sistrunk procedure are known to be minor ones and wound-related. 15 In our study, we report no complications in 30/40 of patients who underwent Sistrunk operation, and in 39 out of the 49 patients in the total cohort. Moreover, other complications were related to infection and no statistically significant difference was found among the different types of surgery.…”

“…4 Surgery in the form of a classic Sistrunk procedure is the preferred method of management. 20 Kim et al reported reduction in the volume of the cyst in 81.3% of patients undergoing ethanol sclerotherapy ablation. 21 The same authors reported that 41.4% of thyroglossal duct cysts treated with OK-432 showed complete regression.…”

“…While infrequent, primary occurrence can also present in adulthood, with an even distribution between males and females. 1 A majority of initial presentations are asymptomatic, but a TGDC may be complicated by infection or fistula formation. Patients who experience these complications usually present with dysphagia, throat pain, or tender neck swelling.…”

“…Predominant occurrence is in the first decade of life with physical exam findings of a midline mobile mass at the level of the hyoid. 1 Most cases are managed with surgical excision in childhood due to possibility of malignancy. While various surgical techniques aimed at removal have been previously described, risk of recurrence still remains high with an average of 11% of individuals experiencing this complication.…”
