Preservation of the Urethral Plate in Hypospadias Repair: Extended Applications and Further Experience with the Onlay Island Flap Urethroplasty

Abstract: The onlay island flap urethroplasty, a variant of the transverse preputial (tubularized) island flap, was originally described for repair of anterior hypospadias. However, many cases of mid and proximal hypospadias have a well developed urethral plate and exhibit little or no chordee after release of skin tethering. Patients with this combination of findings are ideal candidates for onlay island flap urethroplasty regardless of initial meatal position. During the last 5 years the onlay island flap has been use… Show more

“…The evolution of this concept required numerous experiences of dividing the urethral plate and performing extensive ventral corporeal dissection, only to have significant penile curvature persist with artificial erection. 11,12 Our results show that if the preoperative VC is less than 45 degrees, significant improvement should be expected after degloving and about 30% will convert to a straight penis. Even patients with proximal hypospadias and severe VC have a 40% chance of improving after degloving and a 2% chance of requiring no further treatment.…”

“…Decreased rate of fistula formation attributed to i. Preservation of the urethral plate Urethral plate preservation has been reported to be associated with less number of complications [11] . Every effort should be made to preserve the urethral plate during orthoplasty minimizing the need to use tubularized preputial island flaps and expanding the application of onlay procedures.…”

“…However, in as many as 10% of cases, maintaining the integrity of this urethral plate is practically impossible, requiring its removal for the straightening of the penis (4,5). Once the urethral plate is removed, most authors justify that surgery should be done in two steps (two-stages).…”
