Pretreatment Neutrophil–Lymphocyte Ratio

Abstract: The neutrophil-to-lymphocyte ratio (NLR) has been shown to be associated with prognosis in various types of cancer. We evaluated pretreatment NLR as a predictor of poor prognosis in patients with hepatocellular carcinoma (HCC), and we compared the prognostic value of NLR with other prognostic scores. We retrospectively analyzed 825 patients diagnosed with HCC between October 2008 and May 2012. Baseline data, including the NLR and the Child–Pugh class or Model for End-Stage Liver Disease (MELD) score, were recor… Show more

“…Recently, NLRs have also been used to help differentiate primary breast carcinoma from benign proliferative breast disease [97]. NLR has also recently been applied for early detection and as a prognostic marker in gastric cancer [98], early diagnosis of ovarian cancer [99], and prognosis and survival prediction in colon cancer [100] and HCC [101], among others. The use of NLRs and other systemic inflammation markers could help not only aid in treatment choices and prognostic predictions, but also potentially enhance early detection of cancer.…”

“…Moreover, 70-90 % of HCCs are a result of cirrhosis [ 6 , 7 ]. In recent studies, inflammation-based prognostic scores, such as the combination of albumin and lymphocyte counts used in the prognostic nutritional index (PNI) [ 5 , 8 ], the combination of neutrophil and lymphocyte counts in the neutrophil/ lymphocyte ratio (NLR) [ 9 11 ], and the combination of platelet (PLT) and lymphocyte counts in the PLT/lymphocyte ratio (PLR) [ 12 ], have proved valuable in HCC prediction. In addition, recent studies [ 13 15 ] suggest that a simple and accurate biochemical marker of liver fibrosis and cirrhosis, i.e., the aspartate aminotransferase (AST)/ PLT count ratio index (APRI), may be 1) an indicator of postoperative prognosis in early stage hepatitis B (HBV)-related HCC patients, or 2) a marker of HCC risk in HBV patients,.…”

“…Recent data have shown that inflammation is a critical component of tumor progression, and it is associated with a poor prognosis in various tumors, as an oncogenic change induces an inflammatory microenvironment that promotes the development of tumours. 7 11 Studies have shown that a high level of neutrophils is associated with angiogenesis, which plays an important role in the growth and metastasis of malignancy. Furthermore, DNA damage and tumor metastasis suppress lymphocyte activity through the upregulation of cytokines that counteract the antitumor immune response.…”
